Postpartum Care Plan
Overview
Below is a small sample of the postpartum care plan. Each plan is individualized to address your specific needs. We will focus on a few categories each visit to be productive but not overwhelming. Plans can be done prenatally or postnatally.
Mental Health
Sharing your birthing or adoption story, family history, hopes and dreams. Whatever you would like to process, I am a non-judgmental ear here to listen and refer to outside help if needed.
Family
Starting with the most basic unit, the nuclear family, and working our way out. What support do you have in those first six weeks (and beyond)? What would you find most valuable from those that can visit physically, and those that can not.
Resources
How will you track feedings, sleep, and appointments? On average, how much are newborns supposed to eat and sleep? What types of schedule or routine fits the needs of your baby and family?
The main goal is to care for the family. I can care for baby while you get a shower, nap, reconnect with your partner, get a much needed walk etc. I have been working with children of multiple ages for over 15 years.
